We, Cardiomo Care, Inc., a Delaware corporation doing business as Cardiomo (hereinafter referred to as the “Cardiomo” or “We”) explain in this Privacy Statement what information we receive and collect from and about you, a user of our Cardiomo device or other device (hereinafter referred to as the “Device”), mobile or web application (hereinafter referred to as the “App”), our mobile sites and websites that you can access with a Device and App (hereinafter referred to as the “site”), and/or updates to any of the foregoing (collectively referred to as the “Service”). This Statement does not cover any other information that we collect, including, without limitation, data that we collect offline or via our corporate website.
Information We Collect
Information you actively provide (Personal Information): When you download our App, register an account, connect your Device to your account, use the Service or send us requests, we may ask you to provide your first and last name, email address, postal addresses, account name, password, photo, gender, height, weight, and date of birth. Other information you may choose to input includes your diseases, your blood pressure, your mood, and other activities.
Information you automatically provide: When you use or sync your Device, it automatically transmits activity and physical information to us including, but not limited to, EKG/ECG signal and other biometric data, detailed physical information based on monitoring your micro movements, including when you are asleep, when you are awake, when you are idle, as well as your activity intensity and duration. This data is converted into information such as vital energy level, heart rate variability, cardio stress level, tension\relaxation level, your sleep patterns, activities undertaken and your trends and progress. This data can also provide information on certain conditions you may have.
Non-personal information: We also collect data in a form that does not, on its own, permit direct association with any specific individual. We may collect, use, transfer, and disclose non-personal information for any purpose. When you use our apps you transfer information about your Device, including your device type, manufacturer, model, and operating system; your device ID; and the version of your app, language, referrer URL, and the time zone where you use Device. You also transmit information about how you use the app and interact with the pages and features, your activities on our site and a time-stamped log of data exchanges. Your Device and App may also transmit precise location data.
Combinations: We may combine information we receive through the Service with information we receive about you from other sources, for example, via our websites, requests you may send us, or promotional campaigns.
Use of Information
Cardiomo uses the information it collects for analytical, business and operational purposes in the following ways:
1. to ensure operability of and provide updates for the Device, Software & Services and deliver existing and new functionality and features to you;
2. to improve and customize the Device, Software & Services;
3. to provide you with personalized content, including through the delivery of targeted interest-based advertisements from us or our partners;
4. to maintain and improve the quality and operation of the Device, Software & Services;
5. to notify you of Device, Software & Services updates;
6. to provide you with support and handle your complaints and/or feedback;
7. to take any action in case of a dispute involving you in connection with the Device, Software & Services;
8. to enforce the terms and conditions applicable to the Device, Software & Services and to prevent unlawful activities and misuse thereof;
9. to comply and satisfy any applicable law regulation, legal process or enforceable governmental request.
Several areas of the Site use ‘cookies’ ─ small text files that are placed on the user’s computer and stored by the browser. They are used by Cardiomo to provide a more user-friendly, effective and secure service. Cookies also enable Cardiomo to identify a user’s browser and offer services to such user. Cookies do not contain any personally identifiable information. You can prevent the installation of cookies by entering the corresponding settings in your browser software. However, please note that in this case you may not be able to use the complete range of functions of the Cardiomo platform to its full extent.
Sharing of Information
We do not rent, sell or otherwise share your individual personal information with third parties, except as follows:

We use affiliated and unaffiliated service providers all over the world that help us deliver our service and run our business subject to confidentiality agreements. For example, we use third party data analytics platforms to help us understand, among other things, server load and app behavior, in information processing, managing and enhancing customer data, assessing your interest in our products and services, and conducting customer research or satisfaction surveys. These companies are obligated to protect your information.
We share aggregated usage statistics that cannot be used to identify you individually, for example through data stories on our blog (http://cardiomo.com/blog/) or through the media.
We may share your personal information for the purposes of a business deal (or negotiation of a business deal) involving sale or transfer of all or a part of our business or assets. These deals can include a merger, financing, acquisition, or bankruptcy transaction or proceeding.
Choices about Your Information
You can opt-out of promotional emails from us by following the unsubscribe instructions provided in the email you receive, or by contacting us directly (please see contact information below). If you opt-out, we may still send you non-marketing emails, including emails about your accounts and our business deals with you.

You ensure that your Personal Information is accurate, complete, and up-to-date by providing us with this information (e.g. log in your account).
